Value-Based Care is a healthcare delivery model in which providers are more focused on preventive care, and compensated, to some extent, on patient health outcomes rather than strictly compensated on the basis of fee for service patient interactions or encounters.

Under value-based care arrangements, Physicians and Advanced Practice Providers are rewarded for helping patients improve their health, reduce the effects and incidence of chronic disease, and live healthier, higher quality lives in an evidence-based way. The "Value Proposition" in value-based healthcare is derived from measuring health outcomes against the cost of delivering those outcomes. Enhanced Value (i.e., Value-Based Care) for Medicare is realized and shared with Practice Providers when improved health outcomes are produced for patients in a prospective clinical care environment of neutral or lower costs.  

VBC (Value-Based Care) Services was created to support long-term care practice groups who have a desire to change the trajectory of healthcare by participating in innovative, value-based healthcare programs sponsored by Medicare.
